
'Charese Eady, the Telehealth Coordinator for Brooks Home Health, shares some information regarding our Brooks Connect Telehealth Platform. Brooks Home Health has used our current platform as an added intervention to help reduce the rehospitalization rate since December 2018. Patients with diagnoses of CHF, COPD, uncontrolled hypo or hypertension, failure to thrive and recurrent falls are among our telehealth demographic. In order to install Brooks Connect in a patient\'s home, we must have a physician\'s order for telehealth. One of the many features of Brooks Connect includes the ability for both caregivers and physicians to have access to the patient\'s metrics, such as vital signs and survey responses. Some of the features of Brooks Connect are video chat, text messaging, educational videos based on diagnoses of CHF or COPD, quizzes, dietary information and imaging. The device is also programmed in 15 different languages and is bluetooth capable, which means the patient does not have to worry about having the equipment connected to a landline.
